Healthy communities plan


The healthy communities plan has been developed by the Upper Hume Primary Care Partnership in partnership with our 27 member agencies and sets out our priorities for the next three years. 

These evidence-based priorities have been developed from agencies' own consultations with their constituents, consultations with service providers and data relating  to demographics, health and social factors.

This healthy communities plan will, over time, form the overarching plan for our catchments' health and well being.  Each individual agency's own corporate plans will be integrated and correlated with this healthy communities plan. It is anticipated that such an approach will also incorporate other plans such as the municipal public fealth plans, community safety plans, community mental health plans, drug and alcohol plans, and so forth.

We believe that we have developed a model for sustainability.
